How do people reconcile bad things happening to good people with their belief in karma?

I don't mean karma in the original sense (your actions affect your next life), but in the "What goes around comes around." sense. It seems to me that people pick and choose when to attribute an event to karma (i.e. A mother of four dying in a car crash is an accident, but tripping after calling somebody clumsy is karma).
